Your First Visit: Please print and review the forms below and bring to your first session. Prior to our first meeting, I will call you for a 10-15 minute consultation to clarify the scope of your current clinical needs.
Insurance Coverage: I am an out of network provider on all insurance panels. I can provide receipts for submission to your insurance company upon request. You can find out about your out of network benefits, deductibles, and mental health coverage by calling the number on the back of your insurance card.
Payment: Payment accepted at the end of each session with credit card.
